Title: Toggle username

The auto-populate function is nice, but there's a sight annoyance with it, the username. It seems silly to have the username field if a web-site happens to not use a field name that PWM can detect automatically, and as a result, you might need to enter the username twice because you didn't noticed offhand the username wasn't filled in. I would like some way to actually add to the username list for an account (Don't want it global though)

Also, shouldn't the auto-poplate values override the username value, in case for some reason you wanted it to be different, for the forms that use a field name PWM already detects as a username. If so, I'll post a bug report in the bug thread.

Note: The term 'username' means the part of the login that identify the user, even if it's really just a credit card number, phone number or something like that.
