Right now, I don't have access to the form that this issue appears on, but on a site my job uses for time sheets, there's a part that requires a pin code (which I'm using) and the field is a normal password field. A recent change (which is nice in itself as the time sheet page doesn't seem like a slapped on feature to the site it's on now) causes it to appear in a frame set, and when that change occurred, PasswordMaker stopped working.
Hopefully next week I'll be able to pull out a test page that can be use to find why this issue popped-up again.

Alright, I think I know what happened. The bug deal with pages that are three levels deep in frames, not the standard two levels. After Christmas, I'll post a test page if you need more help.
